What the source explains
This companion to OSHA’s guidance translates programme components into examples from healthcare organisations. It is useful for understanding the work between writing a policy and maintaining it in practice. The document is a historical US implementation resource, and its examples should be assessed against current local circumstances. Purchasing an alarm or publishing a policy is not by itself evidence that the response arrangements function.
Using this reading in your hospital
Choose one existing policy and trace its practical path: how a worker learns it, uses it, receives help and sees a problem corrected. Where the path breaks, write down the responsible role and the next action instead of adding another general statement of intent.

Can the committee demonstrate one complete journey from a reported hazard to a checked improvement?
